Respected Parents,
Kindly follow the instructions given below while filling up the application form of Nursery.
- The form should be filled with neat and clear handwriting with utmost care.
- The child’s name, father’s name, surname and birth date of the child should be filled as mentioned in the birth certificate. No changes will be made thereafter anywhere in the form.
- Stick the passport size photograph of the father, mother and child on the form. (Do not staple the photograph).
- Attach the photo copy of the marriage certificate of parents.
- Kindly attach the attested photo copy of the birth certificate issued by the Municipal Corporation or the valid certificate issued by the Gram Panchayat.
- Do not attach the original birth certificate.
- The caste should be mentioned clearly in case of SC/ST/OBC in the application form.
- Attach the true copy of the caste certificate.
- Kindly attach the xerox copy of the Report Card of the last academic year of the sibling studying in this school along with the application form.
- Give the details of your own child studying in this school. Do not give the details of any other child of your relative studying in this school.
- Form should be filled in the medium of language (Gujarati/English) which you want to take admission in.
